Responsibilities
- Performs physical therapy evaluations and develops a plan of care that is appropriate for the diagnosis and age of patient
- Performs physical therapy patient treatment as per the plan of care and modifies treatment plan based on ongoing assessment of patient condition
- Demonstrates an understanding of the operation and indications for commonly used modalities and equipment
- Documents evaluations, daily notes, progress notes, and discharge notes in a timely manner; therapy charges are supported by this documentation
- Communicates and documents necessary information appropriately with patients, families, physicians, and interdisciplinary team members
- Performs other duties as assigned
- Completed department-specific orientation and annual competencies for this position
Requirements
- Bachelors Degree required
- Masters Degree preferred
- Works well under pressure preferred
- BLS certification, valid Wisconsin driver’s license, license to practice physical therapy in Wisconsin required
