Job Description

Hospice patient care managers at Transitions Care oversee teams to ensure compliant, safe nursing care in homes, supporting delivery of 24/7 services that adhere to regulations and provide emotional focus for patients facing end-of-life challenges.

Transitions Care operates as a national hospice provider, delivering home-based physical, emotional, and spiritual support to transform patient and family experiences.

Managers review documentation for compliance, assist with policies, conduct audits, compile quality data, and participate in interdisciplinary initiatives, ensuring plans meet regulatory standards. They maintain knowledge of CMS and accrediting requirements.

Compensation includes competitive salary, medical/dental/vision insurance, life insurance, 401(k) matching, company equipment, and paid time off. Requires BSN preferred, RN license, current BLS certification, valid driver's license, three years hospice experience, and one year field nursing. Involves staff meetings and ethical conduct.

Responsibilities

  • Responsible for team compliance with policies and procedures as well as compliance with local, state, and federal regulatory requirements governing the practice of the assigned team
  • Reviews admission documentation for appropriateness of care and consistency of documentation in coordination with the Director of Nursing
  • Assists the Administrator/Director of Nursing with adherence to the policy and procedure manuals that establish and support the highest possible quality of care to patients and families
  • Serves as a contributing member for Interdisciplinary Group, Quality Assurance Performance Improvement initiatives, including completion of chart audits and strategic agency planning
  • Ensures that the plan of care meets all regulatory requirements including appropriate personalization based on the patient’s initial and ongoing assessment
  • Reviews hospice팀 death, discharge, transfer, revocation, and level of care change documentation ensuring all regulatory components are met
  • Ensures all appropriate external documentation is obtained including but not limited to: History & physicals, diagnostic imaging, lab work (as available), power of attorney (POA) paperwork, and POLST paperwork
  • Works collaboratively with the field staff to assure documentation is complete and consistent with care and reflects legal requirements
  • Compiles, trends, and reports quality data in the following areas: patient care, safety, risk management, infection control, outcomes, and patient satisfaction
  • Ensures compliance with CMS submission requirement (HIS)
  • Maintains current knowledge related to Medicare Conditions of Participation and accrediting organization standards to ensure that the organization is compliant with state, federal and accreditation guidelines, as appropriate
  • Participate in staff meetings, department meetings, team meetings, briefings, in-services, and other related activities as needed
  • Conduct all business activities in a professional and ethical manner

Requirements

  • Graduate of an accredited school of nursing
  • BSN preferred
  • Valid license as a Registered Nurse in the state of employment
  • Certification as a Certified Hospice and Palliative Care Nurse (CHPN) preferred
  • Current Basic Life Support Certification
  • Valid Driver’s License & active car insurance
  • Minimum of three (3) years of hospice experience required
  • Minimum one-year of field-based current nursing experience is recommended
  • Good written and verbal communication skills
  • Sensitive to the needs of terminally ill patients and families and one's own feelings about dying and death
  • Excellent time management and organizational skills are essential
  • Critical thinking skills and ability to solve problems
  • Strong computer skills are required, such as, email, and prior experience working with electronic medical records
