Job Description

As a hospice social worker, you provide emotional and logistical support to patients and families during end-of-life care, ensuring affairs like finances and funerals are in order, and guiding them through grief. The role involves traveling to patient homes, assisted living facilities, or skilled nursing settings to offer tailored support at critical times.

VITAS Healthcare leads end-of-life care in the U.S., offering hospice services to patients with terminal illnesses across 15+ states. The organization supports career growth in caring roles, partnering with interdisciplinary teams to deliver comprehensive care.

Activities include conducting psychosocial assessments, providing grief counseling, advocating for patient needs, coordinating community resources, and documenting in electronic health records. You use tools like patient management software and communication devices, interacting with RNs, physicians, chaplains, patients, families, and community agencies. A challenge is maintaining patient confidentiality and welfare amid emotional demands in diverse home settings.

The hourly wage is $25-$27, work in patient homes or facilities with travel required, flexible schedule including occasionally weeknights or weekends, reliable transportation and valid license needed, and benefits like competitive compensation, health, dental, vision, life insurance, 401(k) with company match, cancer and critical illness benefits, tuition reimbursement, paid time off, employee assistance program, and legal services. Requirements

  • Master's of Social Work from an accredited school
  • At least one year social work experience in healthcare
  • State-required license and/or certification
  • Thorough knowledge of community resources
