Social Worker

As a hospice social worker, you provide emotional and logistical support to patients and families during end-of-life care, ensuring affairs like finances and funerals are in order, and guiding them through grief. The role involves traveling to patient homes, assisted living facilities, or skilled nursing settings to offer tailored support at critical times.

VITAS Healthcare leads end-of-life care in the U.S., offering hospice services to patients with terminal illnesses across 15+ states. The organization supports career growth in caring roles, partnering with interdisciplinary teams to deliver comprehensive care.

Activities include conducting psychosocial assessments, providing grief counseling, advocating for patient needs, coordinating community resources, and documenting in electronic health records.
