Responsibilities
- Assist team in understanding social factors for client health

Requirements
- Master’s in Social Work with 2+ years community-based experience
- Accreditation by Council on Social Work Education
- Licensed Social Worker
- Preferred: previous home healthcare experience
- Ability to travel for cases
- Strong assessment, listening, communication skills
