Job Description

Hospice chaplains at Transitions Care provide non-denominational spiritual support to patients, families, and caregivers in home settings, coordinating with clergy to ensure emotional comfort during end-of-life care that allows focus on living with access to 24/7 services.

Transitions Care delivers hospice services nationwide, transforming experiences for patients and families through physical, emotional, and spiritual care in homes.

Chaplains assess needs, develop care plans, conduct consultations, network with faith communities, provide funeral services, and maintain records, participating in memorials and interdisciplinary meetings. They handle on-call duties and network across service areas.

As a PRN role, compensation includes competitive salary, 401(k) matching, and company equipment; schedules involve local travel, evenings, and weekends. Benefits are not further detailed; interfaith settings and home health experience preferred. No on-site location specified.

Responsibilities

  • Assess spiritual needs of patients/families/caregivers in a timely manner, and provide consultation to patients, families, caregivers and team members in a non-denominational manner or as requested by patients/families/caregivers and communicate to the Patient Care Manager and staff
  • Contact patient's/family's preferred spiritual advisor to ensure ongoing involvement and to invite participation in interdisciplinary care planning when requested by team member or patient/family member
  • Contact clergy or spiritual counselors to assist in spiritual care of patients and family members
  • Develop and document plan for spiritual care, in conjunction with the interdisciplinary team when requested by team member or patient/family member
  • Evaluate and revise plan for spiritual care of patients/families/caregivers with the interdisciplinary team on an ongoing basis
  • Provide consultation, education, and support to the interdisciplinary team on spiritual care
  • Establish networks with faith communities throughout the service area for the purposes of teaching about the hospice philosophy and enlisting aid in providing spiritual support
  • Provide funeral or memorial services for patients as requested
  • Conduct memorial services in nursing homes as requested by Transitions Hospice management
  • Conduct or offer not less than one memorial service per year for family members of Transitions' bereavement program
  • Assure proper maintenance of records as prescribed and in compliance with local, state and federal laws
  • Participate in evening/weekend on-call as required, conducting on-call services in a clinically competent and responsive manner
  • Participate in staff meetings, department meetings, team meetings, briefings, in-services, committees, and other related activities as needed
  • Interact with all staff in a positive and motivational fashion supporting the Company's mission
  • Conduct all business activities in a professional and ethical manner

Requirements

  • Graduate of accredited seminary or school of theology (B.D., M.Div. or equivalent theological degree), or degree in counseling, or equivalent experience
  • Experience working with death and dying, individual/family/caregiver
  • Ability to work as a member of an interdisciplinary team within an interdisciplinary setting
  • Comfort in an interfaith setting
  • Ability to accept different lifestyles, cultures, beliefs and values
  • Ability to network with community clergy and congregations
  • Knowledge of and commitment to Hospice philosophy of care
  • Hospice/home health experience preferred
  • Compliance with accepted professional standards and practices
  • Demonstrate excellent observation and communication skills
  • Self-directed with the ability to work with little supervision
  • Flexible and cooperative in fulfilling all obligations
