Salud Family Health serves communities in southern New Mexico, providing medical, dental, and behavioral health services under a medical home model with team-based care. The organization treats patients regardless of insurance status or ability to pay, reaching thousands yearly to improve broad access. Salud collaborates with pharmacists, dentists, and behavioral specialists to coordinate treatment plans that integrate primary care, pharmacies, and community resources.

Care managers interact with patients to facilitate self-management and behavior change through home visits and clinic consultations. They assist in risk stratification by reviewing electronic records and identifying ACC-enrolled patients needing intensive support. Managers develop care programs including referrals to housing assistance and nutrition programs, then follow up via phone calls and in-person check-ins. Work occurs in team-based settings, collaborating with providers across departments on care milestones and patient goals. A primary challenge includes managing high caseloads of vulnerable patients while documenting accurately in health records systems like the one used for Medicaid applications.

Compensation ranges from $20 to $23 per hour for a full-time role working 8 hours daily in a five-day week from Monday to Friday. Work locations include Salud clinics across southern New Mexico without specific city listings, offering no travel requirements beyond local community outreach. Schedules include 5 holidays, 10 paid sick days, and up to 8 accrued sick days annually, with generous vacation benefits starting at 10 paid days. Responsibilities

  • Assist in risk stratification of high-risk Salud patients, with an emphasis on patients enrolled in the ACC
  • Works in conjunction with other Medical Providers, Dental Providers, Behavioral Health Providers, and all other staff to ensure that the patient obtains the highest standard of integrated healthcare
  • Make initial contact with patients assigned to the ACC
  • Assist in assessing patient’s medical, dental, behavioral, and social needs
  • Develop a comprehensive care program for patients, with referrals to internal and external resources
  • Provide care management, follow-up, and support services to high-risk Salud patients, with an emphasis on ACC patients
  • Follows procedures for the electronic health records system
  • Provide ongoing individualized support to patients in the ACC

Requirements

  • Bachelor's Degree in social work or a health-related field preferred
  • Experience in care management, patient navigation, or social work preferred
  • Experience in a community health setting or with underserved populations preferred
  • Computer knowledge and skills, including proficiency in Microsoft Office programs (Access, Excel, PowerPoint, Word)
  • Bilingual English/Spanish required
