Job Description
Medical social workers conduct per diem visits to provide social evaluations and interventions for clients in Fitchburg, Leominster, Lunenburg, Westford, and surrounding towns. Clients receive referrals to community resources addressing practical and environmental needs for improved health, safety, and independence. Advocates navigate patients through the healthcare system while participating in multidisciplinary teams for total care plans.
BAYADA Home Health Care delivers skilled nursing and rehabilitative services as an accredited and licensed provider. Established for over 50 years, the organization supports clients in receiving necessary medical care at home through a multi-disciplinary approach of health professionals.
Key tasks include making visits to patients to assess needs and develop intervention plans based on evaluations. Workers identify community referrals for client and family support, acting as advocates during system navigation. They participate in development of care plans with clinicians, maintain contracts with agencies as resources, and educate on community resources using instructional materials.
The per diem role includes flexible schedules across Fitchburg, Leominster, Lunenburg, Westford, and surrounding towns. Pay ranges from $63.00 to $68.00 per point depending on qualifications. Responsibilities
- Make home visits to clients in designated geographic territories
- Provide clinical social evaluations and plan interventions based on findings
- Identify community resource referrals for practical and environmental needs
- Act as advocate for client and family navigating community system
- Participate in total plan of care with multidisciplinary team
- Develop and maintain contracts with public and private agencies
- Educate patients, family, and representatives on community resources
- Accurately document observations, interventions, and evaluations on touch pad tablet
Requirements
- Master’s Degree in Social Work
- Licensed Social Worker (LCSW or LICSW) in Massachusetts
