This role coordinates services for individuals in recovery from mental illness and substance use. The position identifies resources for treatment, housing, and rehab needs. By advocating for clients, it improves quality of life and stability. Teaching living skills supports independent futures effectively. Managing benefit applications and budgeting enhances financial security. Supporting medication access aligns with program requirements strictly.

Developing individualized service plans based on client goals collaboratively. Coordinating access to mental health, substance use, physical health resources. Providing community support and home-based assistance judiciously. Participating in case reviews for service quality and stability. Maintaining housing linkages through landlord relations and lease support. Assisting with applications for benefits and payee coordination seamlessly.
High school diploma with 4-6 years human services experience required. Associate degree with 2-4 years or bachelor's with 1-3 years. Valid NYS driver's license and personal vehicle access.
Proficiency in Microsoft Word, basic computer literacy, database usage. Knowledge of psychiatric and chemical dependency support.
Work environment includes office or household settings with ad-hoc evenings. Frequent travel and transport demanded physically.
Physical demands involve sitting, handling paperwork, computer use, communication effective. Occasional lifting to 30 pounds, walking, standing, stair climbing.

Requirements

- Proficiency in Microsoft Word, basic computer literacy
