This role provides care management services for at-risk patients in acute settings, ensuring right care delivery. It addresses care coordination needs by assessing, planning, and advocating for patients. The nurse coordinates resources, reviews utilization, and documents interventions for optimal outcomes.

Work involves assessing patient needs, coordinating care and resources, performing utilization reviews, planning discharges, documenting, and advocating. Interactions with patients, families, and multidisciplinary teams. One challenge is managing care in an acute environment while complying with regulations without on-site weekends.

Requirements
- Associate's Degree in Nursing.
- Washington Registered Nurse License upon hire.
- 2 years acute care experience in Medical Surgical or Inpatient setting.
- IRR or annual competency testing in Utilization Review.
- Bachelor’s Degree in Nursing or higher within 3 years preferred.
- National Certification in specialty preferred.
- 1 year experience in care management or utilization review preferred.
