Job Description
As a clinical liaison, registered nurses coordinate home health services by assessing patient needs during hospitalizations, aiding smooth recoveries. They educate hospital staff on offerings like infusion and equipment services, act as resources for discharge planning, and ensure patients transition effectively to community-based care.
Providence serves over 50 hospitals and 1,000 clinics across seven western states, supporting more than 120,000 caregivers in comprehensive health services. The organization emphasizes whole-person care and community support, continuing a tradition of serving vulnerable populations since 1856.
In this capacity, nurses represent Providence at three hospitals, arranging referrals for services such as home infusion using electronic coordination tools. They build relationships with hospital staff, provide education via meetings, and offer clinical insights on discharge plans. Work involves strong communication, frequent travel between sites, and collaboration with interdisciplinary teams including physicians and case managers.

Requirements
- Oregon Registered Nurse License
- National Provider BLS - American Heart Association
- 1 year full time clinical experience
- Experience in home health, hospice, home infusion, or discharge planning preferred
